Medical benefits are provided through Anthem Blue Cross Blue Shield for all locations and Kaiser exclusively for California associates. When comparing plans, be sure to consider factors like premiums, deductibles, co-payments, and out-of-pocket costs. Once selected, your choice will stay in effect for the entire 2025 plan year, unless you experience a Qualifying Life Event.

Keep in mind premium contributions for medical coverage are deducted from your paycheck on a pre-tax basis. Your level of coverage determines your contributions (see your 2025 Benefits Guide for biweekly contributions).
If you have satisfied the Wellness requirement for 2025, you will receive a $600 discount applied to your medical premium.
Important to Consider
Anthem Blue Cross Blue Shield Options
A Preferred Provider Organization (PPO) plan has higher premiums but lower costs when you need care. You can choose from a network of providers with set co-pays. If you expect more medical care this year or have a chronic condition, a PPO might be a good fit. Anthem Blue Cross Blue Shield offers two PPO options: a premium plan and a standard plan, both FSA-eligible.
A High Deductible Health Plan (HDHP) has lower premiums, but you’ll pay the full cost for non-preventive services until you meet your deductible. If you mainly need preventive care (covered at 100%), an HDHP may work best. Anthem offers two HSA-eligible HDHP options.

Kaiser (California) Options
You have the option of a premium or standard Health Maintenance Organization (HMO) plan. Both require you to use in-network providers, with coverage through co-pays or coinsurance. Out-of-network care is not covered, and in-network providers offer discounted rates for services.
